Pier40Baseball Day Camp is licensed by the City of New York Department of Health and Mental Hygiene.
We are required to have current health forms for all campers on file. Last year's form is not acceptable and
campers cannot attend until we have the form. You can use a photocopy of any form used for school or another camp,
or a standard form provided by your physician. You can also use the form available below.
